Disagree again. The pitcher was in "the stretch", preliminary movement, when the runner broke to 3rd therefore Rule 8.01 applies.

with the pitcher in the set position, does stepping off the rubber at the start of a pickoff move necessarily have to be behind the rubber? Can he step off in front or to the side of the rubber as well?

He can be on the rubber but he has to pivot, or "break" from the rubber. If he steps off, it has to be BEHIND the rubber.
